Where Is the Problem?

More than 70 million people in Pakistan are living below the poverty line due to which they are unable to undergo proper health treatment if they experience any disease. Estimated amount of surgery backlog in Pakistan is in millions every year.

  • There are not many health insurance plans for underprivileged community. They have to pay out of pocket if they need any medical treatment but due to the limited or no resources, majority is unable to undergo surgical treatment.
  • Beyond the cost of the surgical procedure, they might have to pay for pre and post-operative expenses which includes medical tests, medications, hospital stay and other expenses. It is almost impossible for someone who earns less than $2 a day to pay hefty amount for medical relief.
  • There is lack of advanced medical facilities and medical professionals due to which lower middle class families are unable to avail treatment facilities.
  • There are a limited number of beds in hospitals but the number of patients visiting these hospitals is a lot more.
  • Every second patient including a child is put on a long waiting list and during the waiting period, the patient’s health further deteriorates. In most of the cases, patients expire until their turn comes.
  • There is no advanced healthcare services in slums due to which majority of people have to rely on quacks in their areas.
  • Private hospitals are out of reach for deprived community due to the high costs of consultation fees and surgical procedure.

1-  Anaesthesiology

Anesthesia Department at Shaukat Khanum Memorial Cancer Hospital and Research Centre provides anesthesia to a wide variety of surgical patients inside the four operating rooms.

Facilities & Services

Anesthesia is also provided in MRI suit, where a state of the art MRI compatible anesthesia machine and monitoring is used for safe conduct of the diagnostic test. Sedation and anesthesia is provided in Endoscopy unit for either Gastro-esophageal or pulmonary interventions. Sedation and anesthesia is also provided for vascular coiling and for neuro-interventional radiology. Ambulance Service

Edhi Land Ambulance Service was initially started by including a second hand Hillman Pickup Truck and that was refurbished into the first ambulance, thereby coining “Poor Patient Ambulance”. Now sixty years after, the Edhi ambulance has reached to the stage of largest fleet of ambulances in the world, thereby providing with a tantalizing number of ambulances—such as 1800 vehicles, all over their country – Pakistan.

Child Adoption Center & Childcare Services

The history of establishing the Edhi child adoption centre and childcare services dates back to 1949. Mrs. Bilquis Edhi is supervising and looking after the caring and feeding of babies and children. For this purpose, exclusive cradles have been placed outside all Edhi Centres, across the country with a view to keep the abandoned and illegitimate babies in these cradles.

Mostly these abandoned newborn babies are provided to the childless couples, who in accordance with their own policies, after ensuring that they are fully deserved and exactly suitable for this noble cause. Bilquis Edhi, after completely going through the background of the couples, and undertaking a rigorous screening process she decides that the couple or the family is precisely suitable for the baby adoption.

On annual basis, Edhi Foundation is giving over 250 babies or children for adoption.  Till to date, over 23,320 babies and children have been provided to the childless couples and families.

About

It is run by a team of highly motivated intellectuals and social workers comprising of Doctors, Engineers, Lawyers, Business Executives and Representatives of various fields of life. Al-Mustafa’s branches are working all over the country from Kashmir to Karachi. They are providing Health, Education, Public Awareness and Emergency Relief Services. Mobile Health Units (MHUs)

The Pakistan Red Crescent Sindh Branch, seeks to provide accessible, affordable and quality health care to the population in rural & inaccessible areas, especially the vulnerable sections. Given the status of public health infrastructure in our State, it is not possible to provide the desired services till the infrastructure is sufficiently upgraded. Factors negatively influencing in delivery of health services through existing public health institutions are as follows:

  • Distance of remote villages from public health institution
  • Geographical barriers to reach out to such pockets
  • Lack of inadequacy in public transportation network
  • Lack of health awareness &health consciousness among people, hinders in accessing health services on their own.
  • No exclusive medical team to reach out to these disadvantaged sections.
  • Mobile Health Unit (MHU) in our health delivery system is designed as an alternative model of rural health care delivery for a specified area with earmarked team of health professionals with supporting equipments & drugs. It is envisaged to provide preventive, promotive & curative health services in the inaccessible areas which are un-served /underserved under usual circumstances.

Provision of a mobile health unit with ambulance has been planned for flood affected areas. PRCS, Sindh Branch is providing basic health care services through field and mobile health structure to the remote and hard to reach areas of Sindh Province.

About

HASWA is a not-for-profit organization working for the thousands of impoverished children in Pakistan who suffer physically from a congenital deformity or injury and do not have access to basic medical care let alone the re-constructive plastic surgery they need to lead normal productive lives.

Working in partnership with volunteers and overseas organizations, HASWA began in 2001 by providing free reconstructive surgery to people, specially children. Born with congenital deformities such as cleft lips or palates, they are often denied normal education because of their appearance and speech impediments.

To provide emotional and financial support to people suffering from congenital defects and physical handicaps specially those who are unable to finance their treatment.

HOPE (Health Oriented Preventive Education) is a NGO (non-governmental organization) based in Karachi, Pakistan, comprising of professionals (Doctors, Social workers, Educationists) as well as grass-root workers. The motto is SELF-SUFFICIENCY and SUSTAINABILITY.

The ultimate aim of HOPE is Community Development through self-sufficiency of the community. The members of HOPE are working along with community volunteers who are organized in Community Based Organizations (CBO’S). All activities in the community are planned in co-ordination with the CBO’S and implemented in the community through the CBO’S. Hence, the community becomes SELF- SUFFICIENT and the community development work is SUSTAINABLE.

Maternal Child Center

Maternal mortality is significantly high in Pakistan. This is chiefly due to pregnancy related complications such as threatened abortion, ante-partum hemorrhage, pre eclampsia, etc. Mothers generally do not go for ante – natal care and most deliveries are conducted at home.

Maternal and Child health Centers have been established in Manzoor Colony, Mujahid Colony and Bilal Colony where ante – natal care is provided to mothers. High-risk pregnancies are identified. Anemia is treated, nutrition advice is given and fetal monitoring is carried out. For this purpose a liaison has been established with the TBA’s in the area. Simple deliveries are conducted in the MCH Centre. In case of complications, the MCH Staff immediately takes the patient to a Tertiary Care Hospital.

Basic Health Unit in HUB

HOPE, in collaboration with Asia Petroleum, started a Basic Health Unit in the HUB area in August 2010. The Health Center is located in a poor area. Majority of people live without electricity, poor hygiene and sanitation facilities and limited clean water. Diseases are common in this area and health facilities are situated far away. HOPE aims to provide much needed basic health care to thousands of residents annually through this BHU.

Mobile Health Unit (MHU)

To cater the much needed health care facilities in the underprivileged areas of Karachi, HOPE organizes a weekly mobile medical camp that covers as many as 26 Goths on a rotational basis. HOPE has been able to carry out this project with the help of generous support from ASIA Petroleum. The area that HOPE’s medical team covers is quite vast ~ from Pipri in the south east of Karachi to the North West touching the borders of Baluchistan at Hub. On each visit  as many as 60 individual patients  consisting of men, women and children are examined

Health Services

Alkhidmat Foundation Pakistan has always been striving to serve the poorest segment of the society. Keeping in the view the situation of poor infrastructure of health sector in Pakistan, they have started a number of projects to improve the situation by provision of facilities for the general public, regardless of their socio-economic status. Alkhidmat Foundation has devised a health services program for establishing and administering Mother and Child Healthcare Centers, Hospitals, Mobile Medical  Camps, Ambulance Service, Diagnostic and Collection Centers and Blood Banks across Pakistan.

Ambulance Service

Due to the increasing numbers of road accidents, terrorist attacks, poor health conditions of masses, terrorism and natural disasters, people are extremely vulnerable to causalities. In such uncertain circumstances, they need a quick access to any sort of medical facility which can decrease the chance of any major loss. In such situation the role of ambulance service has gained major importance. The objective of Alkhidmat Foundation for its ambulance service is to mitigate the severity of accidents or disasters where many people die as a result of unavailability of quick access to hospitals or ambulances arriving too late on the scene. A large number of our ambulances across Pakistan provide appropriate pre-hospital care to the patients and or victims of any emergency situation. They are kitted out with necessary equipment like splints, oxygen, drugs, support of paramedical staff and others.

Medical Camps

Besides its several health and medical facilities, Alkhidmat Foundation Pakistan also arranges mobile medical camps as its out-reach program to provide medical and health facility to those who can’t afford medical services. These camps provide specialized medical staff for the patients at their nearby place. These medical camps provide initial diagnostic facility through qualified medical staff to the patients. Patients are examined and prescribed for initial treatment according to their symptoms and sometimes they are referred to a suitable medical facility.

Free helpline 1023

The free helpline 1023 enables the citizens to reach Alkhidmat Foundation Pakistan’s Ambulance Service from across the country. The countrywide network maintains a track record of 10-minute response time and documentation system.

Conclusion

Doctors at present day are actually better technically at what they’re doing in their specialty compared to the doctors in the same field 40 or 50 years ago. Over this long period of time, medical science has made major advances but the human relationship between a doctor and patient is lost. It used to be a compassionate relationship when the doctor  looked people in the eye and say, “I care about you. We can do this together.”

When an individual becomes a doctor after 6 years of training he or she will never be the same person again because they are committed to do everything in their power to save lives, preserve health and alleviate the suffering of the patients.

In the recent years, however, we have seen that the health care system and services provided by the physicians have deteriorated rapidly. The nation was shocked and taken aback with incidents of terrible callousness leading to deaths of many hospitalized patients often due to wrong diagnosis, wrong medication or careless activities during the surgeries which have raised several questions about the moral and ethical practices of the doctors.

Then we have also seen incidents where some hospitals shamelessly showed reluctance to provide surgical emergency treatments to poor patients because they didn’t have the money to pay for the expensive facilities.

Doctors are at fault for not maintaining high ethical values in their professions but it is the hospital which should be blamed in the first place for turning our healthcare system into a business and its facilities into mere commodities.

A surgeon’s fee is only 10% of the total cost of the surgery but the rest 90% cost is payable for disposables like sterile sheets and gauzes, surgical instruments including implants and staplers, anesthetic drugs, postoperative ICU care including ventilators, IV fluids, use of cardiac monitors, water, electricity and AC charges, salary of OT technicians, nurses, ward boy, resident doctors, hospital staff, security, hospital bed, medicines etc. If a patient fails to pay for the hospital dues the authorities often cut the surgeon’s fees first to make up for their loss!

Hospitals operating across the countries need a little reformation in the way they are run and administered. The hospital’s administrators and CEOs must not lust for  making big bucks. They should hold the high mentality of servitude because the medical profession is the only field in the world which intervenes in a person’s life and death. So medical is the most serious and sensitive field which should be handled only by people who have specialized knowledge about it. Every hospital should think about maintaining a separate fund to provide health care services, medications, and surgical treatments to the poor and needy patients without charging them any fees.

The hospitals should stop pressurizing doctors to over treat the patients, prescribe too many medicines often which are not needed like vitamin supplements to make profit from the hospital pharmacies, or force patients to go for surgeries instead of normal deliveries to earn more money, delaying discharge of patients so that more bills  are padded and other commercial methods or incentives they imply to make profits. Doctors should understand their responsibility and practice high accountability.

Meanwhile, nongovernmental organizations should keep up their humanitarian work and organize free medical camps on a regular basis for the poor patients to treat their diseases and heal their souls. Free medical camps are bringing positive change to the communities. Access to healthcare is a basic human right. The medical camps are filling the void in the country’s healthcare system ensuring equal healthcare and treatment for all.
